Insurance coverage varies by plan. Our office will assist with verifying your benefits and eligibility prior to your first appointment.

Payment

All major credit and debit cards are accepted. Zelle payments are also available for private-pay services.

Payment is due at the time of service unless otherwise arranged.

Cancellation Policy

If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice.

Appointments canceled with less than 24 hours' notice, as well as missed appointments, may be subject to a $75 fee, where permitted by applicable law and insurance requirements.

Additional details regarding attendance and cancellation policies will be provided during the intake process.
